Privacy notice
PDIC collects information that you voluntarily submit through mentorship, shadowing, and email-list forms. This may include your name, email address, school, educational stage, application plans, and the questions or narrative information you provide.
How information is used
- To review and respond to requests.
- To administer mentorship or shadowing communications.
- To send updates when you separately join the email list.
- To maintain form security and prevent spam.
Do not submit sensitive records
Do not submit Social Security numbers, financial-account information, medical records, patient information, or complete transcripts through a general website form. If document review is arranged, PDIC will provide separate instructions.
Service providers
The website uses Netlify for hosting and form processing and may use Mailchimp for voluntary email subscriptions. Those providers process information under their own policies.
